For Patients with Trillium Medicaid
If you are currently enrolled in Trillium Medicaid and listed Sandhills Best Care as your Tailored Care Management (TCM) provider, please contact us at (910) 562-9882 to schedule your assessment and begin receiving services.

If you are enrolled in Trillium Medicaid but have a different provider listed, you may call us to request a provider change and switch to our care management services.

Note: You do not need to be enrolled in Trillium Medicaid to get started — we can assist you with transitioning to Trillium.
 

Important Information for Medicaid Members
To confirm whether you are already enrolled in Tailored Care Management and to identify your current provider, call the Member Services phone number located on your Medicaid card.

You may request for Sandhills Best Care to become your TCM provider during this call.

If you currently do not have insurance, we encourage you to visit our office to complete the necessary forms:

• Sliding Fee Scale Application
   

Note: Both forms must be completed and submitted to determine Medicaid eligibility and potential enrollment.
